03 / Governed documentation
Forms that think before they save.
Form Builder is clinical documentation with medical-necessity logic built into the form itself — not bolted on afterward. The form knows what the payer needs, and it won't let the note go out incomplete.
The design
Documentation that defends itself.
Most denials are documentation failures wearing a billing costume. The note was missing the one field the payer's policy required — and nobody knew until the denial letter arrived. Form Builder moves that check to the moment of documentation.
- Medical-necessity logic in the form — payer policy requirements expressed as form rules, checked as the provider documents.
- Critical fields required before save — the fields a payer needs to see can't be skipped; the form says what's missing and why.
- Key fields stored for reporting — structured capture means the data is reportable later, not trapped in narrative text.
- One choice, filled everywhere — an answer entered once propagates to every form that needs it. No re-typing, no contradictions.
Why it matters
The denial starts at the keyboard.
Cleaner claims, upstream
When the form enforces medical-necessity completeness at documentation time, the claim that follows is built on a note that already answers the payer's questions. Fewer avoidable denials, fewer rebills, less rework for billing.
Providers document once
Smart propagation and governed structure mean the provider answers each question a single time. The form does the assembly; the provider does the medicine.
